Awarded at the end of the 2024–25 financial year, this grant supports Back on Track’s ongoing rehabilitation services for working-age stroke survivors across Chippenham. The organisation provides long-term, holistic support that goes beyond initial hospital-based care, filling a clear gap in provision for younger people recovering from stroke.

The project offers a blend of 1:1 home visits, community-based stroke exercise classes, counselling, psychotherapy, and tailored rehabilitation therapies. It aims to help individuals rebuild physical, cognitive, and emotional resilience while supporting them in regaining independence and reintegrating into community life. Clients are referred by local GPs, the Chippenham Hospital Stroke Unit, social prescribers, and community therapists. The team works closely with local leisure centres and gyms to promote physical activity as part of recovery.

With no other service in Wiltshire offering this kind of long-term, personalised support to younger stroke survivors, Back on Track plays a vital role in improving access to rehabilitation, reducing isolation, and supporting both survivors and their families through the challenges of recovery.
